To start the process, you must contact your Care Manager to ask for an assessment of your needs.

When you book the appointment, it would be very helpful to direct your Care Manager to this website. Not only will they be able to appreciate what you're hoping to achieve, but on our Professional Advisor's Site, they can read some useful information to help them with your assessment.

When you attend the meeting, please take with you the Care Manager Assessment Form. You'll need to print a copy of it from our website, or ask your Care Manager to print a copy of it.
