the opportunity with the person concerned, their parents/carers and the
person they trust (who may be appointed as their Attorney or Receiver following
your positive assessment). All of these people should be directed to our consumer

in order they may understand the opportunity. |

Carry
out an assessment to see if the person would have their needs better met
if they moved from their current accommodation into a home of their own.
Your assessment is needed to access the Income Support that will pay
the mortgage interest and the Housing Benefit to cover the rental portion
of the Shared Ownership Home. |

If
the person has a learning disability, establish
whether the person has sufficient mental capacity to understand the
transaction on their own; whether they have the sufficient mental capacity
to appoint an Attorney; or whether they have insufficient mental capacity
and need to appoint a Receiver |
 |
Identify
a support provider from your existing suppliers or request that we
nominate one |
 |
Arrange
for payment of the fee of £1,500 on completion of the transaction and
arrange for any re-charge, or confirm who we should contact at your Local
Authority to organise this |
